Normative Foundations for Equity-Sensitive Population Health Evaluation Functions

چکیده
Standard models for the evaluation of population health, such as the so-called models of aggregate Quality Adjusted Life Years (QALYs), or aggregate Healthy Years Equivalent (HYEs), are usually criticized on equity grounds. We provide in this paper normative justifications for alternative equitysensitive models, such as the so-called models of multiplicative QALYs, multiplicative HYEs, and generalizations of the two. Our axiomatic approach assumes social preferences over distributions of individual health states experienced in a given period of time. It conveys informational simplicity, as it does not require information about individual preferences on health.

Labonté Identifies Key Issues for Health Promoters in the New World Order; Comment on “Health Promotion in an Age of Normative Equity and Rampant Inequality”
For over 35 years Ronald Labonté has been critically analyzing the state of health promotion in Canada and the world. In 1981, he identified the shortcomings of the groundbreaking Lalonde Report by warning of the seductive appeal of so-called lifestyle approaches to health. Since then, he has left a trail of critical work identifying the barriers to — and opportunities for —health promotion wor...

This paper introduces the rank-dependent quality-adjusted life-years (QALY) model, a new method to aggregate QALYs in economic evaluations of health care. The rank-dependent QALY model permits the formalization of influential concepts of equity in the allocation of health care, such as the fair innings approach, and it includes as special cases many of the social welfare functions that have bee...
متن کاملGlobal Health in the Anthropocene: Moving Beyond Resilience and Capitalism; Comment on “Health Promotion in an Age of Normative Equity and Rampant Inequality”
There has been much reflection on the need for a new understanding of global health and the urgency of a paradigm shift to address global health issues. A crucial question is whether this is still possible in current modes of global governance based on capitalist values. Four reflections are provided. (1) Ecological–centered values must become central in any future global health framework. (2) ...
متن کاملHealth Promotion in an Age of Normative Equity and Rampant Inequality
The world was different when the Ottawa Charter for Health Promotion was released 30 years ago. Concerns over the environment and what we now call the ‘social determinants of health’ were prominent in 1986. But the acceleration of ecological crises and economic inequalities since then, in a more complex and multi-polar world, pose dramatically new challenges for those committed to the original ...
